Root Canal vs. Extraction: How to Decide

When a tooth is badly decayed or infected, you usually have two options: save it with a root canal, or remove it. Here's how dentists weigh the decision.

Why saving the natural tooth is usually preferred

In most cases, dentists recommend root canal treatment over extraction whenever a tooth can realistically be saved. A natural tooth preserves normal chewing function, keeps neighbouring teeth from shifting, and avoids the cost and time of a replacement (implant, bridge, or denture) later.

When a root canal is the right choice

Root canal treatment works well when enough healthy tooth structure remains, the surrounding bone is in reasonably good condition, and the infection hasn't caused extensive damage to the root. After treatment, a crown is often placed to protect and strengthen the tooth long-term.

When extraction becomes the better option

Extraction is usually recommended when a tooth is fractured below the gumline, has severe bone loss around the root, is significantly impacted (as with some wisdom teeth), or when a root canal has already failed and reinfected. In these cases, trying to save the tooth can mean spending more without a reliable long-term outcome.

What replaces an extracted tooth?

If a tooth is extracted, replacement options include a dental implant (a titanium post topped with a custom crown), a fixed bridge anchored to neighbouring teeth, or a removable partial denture. Your dentist will recommend an option based on bone health, budget, and which teeth are missing.
